چکیده

Background Traditionally, non-cuffed tubes have been used for tracheal intubation of children under eleven years of age. The evidence is now strongly supportive of cuffed tubes in this population. Objectives We aimed to ascertain the patterns of tracheal tube usage in paediatric anaesthesia in Ireland. Materials and methods A self-structured questionnaire was distributed at anaesthesia conferences and departments in Ireland between March and August 2012 (n = 231). Results 75% of the 231 respondents routinely used non-cuffed tracheal tubes in these children. Emergency procedures in unfasted children, obesity and planned laparoscopy influenced anaesthetists to use a cuffed rather than a non-cuffed tube(p <0.001). 66% of respondents were unsure if they would use a high-volume low-pressure cuff. Compared to anaesthetists with proportionately lesser paediatric workloads, those with greater paediatric workloads were inclined to use a cuffed tracheal tube with a high-volume low-pressure cuff(p<0.01). Conclusion We concluded that despite plentiful data supporting the use of cuffed tracheal tubes in children their rate of use is low amongst anaesthetists in Ireland. There is considerable uncertainty regarding high-volume low-pressure cuffed tubes.

Cuffed or uncuffed tubes during anaesthesia in infants and small children.

Editor—The large randomized controlled trial of cuffed or non-cuffed tracheal tubes in small children demonstrated that experienced practitioners using exemplary (as opposed to standard) technique had a similar incidence of post-extubation stridor with either tube, and fewer repeat intubations with the cuffed tubes. 1 In the accompanying editorial, 2 it is proposed that paediatric anaesthetists...

Cuffed tubes for infants and children in anaesthesia and intensive care: Why we should change to cuffed tubes in paediatric airway management

Since almost 50 years paediatric intensivists and anaesthetists have used uncuffed tubes in infants and children and have lived with the handicaps of uncuffed tubes. In adult anaesthesia and intensive care cuffed tubes are routinely used since approximately the same time. The paradigm to use uncuffed tubes in infants and children and the condemnation of the cuff are in fact not very logical and...

Prospective randomized controlled multi-centre trial of cuffed or uncuffed endotracheal tubes in small children.

BACKGROUND The use of cuffed tracheal tubes (TTs) in small children is still controversial. The aim of this study was to compare post-extubation morbidity and TT exchange rates when using cuffed vs uncuffed tubes in small children. METHODS Patients aged from birth to 5 yr requiring general anaesthesia with TT intubation were included in 24 European paediatric anaesthesia centres. Patients wer...

Bed Utilisation in an Irish Regional Paediatric Unit – A Cross-Sectional Study Using the Paediatric Appropriateness Evaluation Protocol (PAEP)

Background Increasing demand for limited healthcare resources raises questions about appropriate use of inpatient beds. In the first paediatric bed utilisation study at a regional university centre in Ireland, we conducted a cross-sectional study to audit the utilisation of inpatient beds at the Regional Paediatric Unit (RPU) in University Hospital Limerick (UHL), Limerick, Ireland and also exa...

Uncuffed versus Cuffed Endotracheal Tubes

SPA-APA Meeting San Francisco 2007 1 Uncuffed versus Cuffed Endotracheal Tubes Markus Weiss, MD, Zurich, Switzerland During the last five decades, uncuffed tracheal tubes were recommended for routine use in children aged below 8 years. Cuffed tracheal tubes in patients younger than 8 years of age were only exceptionally used for special indications. In the last 10 years a change in clinical pra...
